Yes, I want to apply for ACE’s Broadband Internet Access. In order to be eligible for ACE’s services, which include broadband Internet access and installation at no charge to my educational organization, I agree that my Internet access will be used for educational and administrative activities. I agree that I understand and accept the following terms and conditions:
	The applicant must be an accredited educational institution recognized by the State of California. ACE reserves the right to review all applications and approve or deny each request based on the requirement that the Internet access service will be used for educational purposes. The Usage License, if approved, shall be for a period of one (1) year from the installation date and may be extended by ACE for additional one (1) year periods at ACE’s sole discretion. ACE has a maximum level of funding for its Broadband Internet Access Education services per calendar year. Once the full amount has been granted for the year, no further applications will be accepted. By submitting this application, I certify that I have read, understand and agree to these criteria.
